The cause of the Challenger event (I cannot use the term "accident" in this context)is rooted in the political compromises made in the post Apollo era and in a contract precurement process rife with political expediency. The problems with the o ring seals were well understood well before the failure on mission 51-L. Many Nasa engineers rode out every launch up to that point with their fingers crossed behind their backs. Sadly the lessons of Challenger were soon forgotton and I believe were instrumental in the destruction of Columbia in 2003. Nature cannot be fooled.
