This can be avoided if you have a limit on the number of revisions you will perform and state VERY CLEARLY up front what that means. Even as low as 1-2 revisions after the proof is accepted is pretty normal. In addition, you can just charge them per revision beyond your stated limit until they're happy.
This does assume that you are a competent designer and aren't starving for work. If you give the customer a **** piece of work and they don't like it, that's a matter of quality standards, not revisions. Similarly, if you need their job really bad, then I guess you should be happy to perform as many revisions as they want.
