Monitoring tools

I like option 1.  That format makes it easier to complete. I also like the idea of recording attendance, achievements and observations.

I would add a section to date when target goals are being met.

I agree about the additions. There can also be a remarks column to comment on the effectiveness on how it is working and if not maybe alternative suggestions. It might time consuming to annotate, but it can benefit both the student and teacher and even other teachers who will be getting those students in the future.
