improving the patient experience

1) ensure GP's feel involved and supported 

2) try to ensure we have the right people at the right time to support. 

3) ensure the team communicates effectively and has a shared plan in which the person and others whom they wish to be are fully invovled. no decision about me without me. 

4) treat people with dignity and respect. 

5) ensure clients have a written escalation plan (beyond a respect form) which has contact details of whom to contact in what circumstances.  

6) enable a focus on quality of life, activities, hobbies

7) ensure safety and comfort are priorities
