This week you will start to consider the practical application of multidisciplinary care. The video will explain what this week is all about.


Whilst you work through the tasks in week 4 consider these focus questions:

  • Who needs to know what?
  • How might you to ensure the key messages from your role are understood and acted upon by the wider team?
  • How is communication organised in your work environment currently?

Recommended reading

The NICE guideline on MND emphasises the importance of coordinated care. But it does not specify who should undertake the coordination. In the field different approaches are taken to coordination and more than one can be effective.

Processes which record patient wishes, such as advance care planning, may support coordination and help ensure a positive experience of care for the person living with MND. Health and social care professionals may find such documents useful to avoid asking repetitious questions of their patient and also to know the interventions either discussed, or carried out, by their MDT colleagues.
