Welcome to week 2!

 This week you will identify the members of an idealised core MDT and network and consider their application within your own practice.

 This video will explain what you will be doing this week.

 


Whilst you work through week 2 consider these focus questions:

  • Who makes up the core MDT and wider network recommended for the effective care of people living with MND?
  • Where, and who, are the members of your own MDT and network?
  • How accessible is the core team in your area?
  • How prompt are referrals to the wider team and how prompt is the action following referral?

Recommendations 1.5.4 and 1.5.5 of the NICE Guideline set out the core MDT and the wider network considered necessary for the effective care of people living with MND. In practice MDTs are organised in many different ways. Some teams operate virtually, with members co-ordinating activity and interventions by various means of communication. Other teams might meet face to face once month to discuss cases. The common theme is that the teams work together to deliver the best care possible to the person living with MND.
