To investigate communication within a multidisciplinary team. 

Task

Please take a look at this document which lays out one way of describing the elements of providing effective care  here (PDF document138.2 KB) .

After you have read it, consider and record:

What communication channels are available to your team?

Consider how your team uses the communication channels at its disposal to deliver these key aspects of communication.

Is this effective? Can you suggest how communication might be improved to ensure that desired outcomes are achieved?
