MND Clinical Nurse Specialist

I have currently taken over the role of a MND Clinical Nurse Specialist in the community and help support with our MND patients and also work closely with members of the MDT who are involved with the patient care to ensure they are well supported. 

I am currently working on a MND project on how we can support our MND patients in the community, through sign posting, supporting with referrals in the hospice for our MDT patients, Emotional and psychological support, advanced care planning. 
Through the project I will looking at how we can support MND patients who might not a hospice involvement but how we can support on an information which can provided to them through the MNDA, sign posting, Looking at grants etc.
 
I am just wondering if anyone can advise on they support their MND patients in a community setting? 

Also has anyone written a SOPS or Policy for support with MND patients, if they could they give some advise or guidance on how to write one.
