Introduction

Hi my name is Charlotte I am a Clinical Nurse Specialist at a hospice and for the last 18 months I have taken over the role of the MND Clinical Nurse Specialist and through my role I have been supporting patients in a community setting. 

Through my job role I work closely with different members of the MDT who are involved in our patient care and ensuring we are supporting our MND patients together. 

I am currently working on a MND Project on how we can support our MND patients in the community and how they can access services in the community or through the hospice. Through the project I will also looking at MND patients who are currently in the community but not under a Hospice and we can help them with any concerns or worries they have.
