Communication

My methods of communication will vary depending on what I need to communicate. 

Methods available are: 

  • Face to face (patient appts / support groups / local MDT meeting) 
  • Virtual (patient appts / Neuro Palliative wider MDT)
  • Email
  • SystmOne third party record sharing
  • SystmOne tasks
  • Telephone
  • Clinic letters / summaries 

If I am pushed for time and need to speak with another member of the MDT regarding something and I can't get hold of them via phone, I will often send a S1 task for their attention. 

I find the Neuro palliative MDT very beneficial as this includes myself as navigator, SLT, Neuro PT & OT, Dietitian, Hospice nurses and sometimes PT, MND advisor, MND CNS from care centre, Adult care services therapist and Palliative care community nurse. It is once a month but a really goof forum to bring things to the meeting and link up together. We also discuss recent RIP's which is helpful to us all. 

I find it beneficial to complete a visit summary with an action plan to the patient, GP and care centre. 

I complete all assessments face to face. Patients have my email address so that they can contact me easily without having to go through the SPA Hub telephone line.
