Occupational therapist

I find you need that one central person, such as a care co-ordinator to be at the centre of all care. They have regular contact with the person/family and can refer to any discipline easily as they have the contacts/details. 

Unfortunately, there is only one person to cover all of our local areas. When she is off, it is down to the neurology team to cover and this causes a lot of issues with communication due to limited time and not knowing who to contact. 

The professions I tend to work most closely with are:

Physio - often joint working on mobility/aids/referrals for orthotics.

SALT - when identifying issues with swallow/speak/excess saliva that is difficult to manage.

Environmental controls - to be able to operate electrical items around the home or use phone/computers. Often completing joint visits. 

Neurologist - MDT meetings to highlight any issues that may be medication related.
