Using the telephone to communicate, arrange appointments, review how thing's are going is generally a good method until the person loses the ability to use a phone in a conventional way. 

This is obviously very upsetting for the person and causes much frustration. Luckily in this day and age, there are many other methods. Devices such as Alexa and Google Home play a part in facilitating phone calls. Many find using their communication aid to instruct Alexa to call OT works well. They can then use their communication aid to speak for them. this tends to be a long process and due to very busy working days, this is not always feasible. I may be in the car when they call or with another patient and I can see how this may be a barrier to communication. 

Communication between professions can be difficult. I often use email however a person may not always respond. I also call but again, people are often with other patients etc and may not respond. This is frustrating as important information needs to be passed on. We do have a monthly MDT on Teams. Sometimes this information cannot wait for this meeting. Speaking on Teams is also quite difficult if there are lots of you. 

I am unsure how to improve this. It is something we discuss regularly within the team but have not found a solution.

I resonated with this reflection Kaye. Joined up working is the ideal, however there are many challenges in practice including those you have outlined above.
