Reflection Week 5 - Case Studies

Was interesting to read the x3 case studies and to hear the view from the videos. 

I think certainly from my point of view because I complete a full hoilistic needs assessment when I meet the patient, I do a lot of onwards referrals and liaising with the wider MDT following that assessment. 

I also think by having good connections with the MDT and good communication it can save work in terms of already making referrals that may have already been done etc
