Care Centre Coordinator

My role is similar to a number I have read here but different. 

I am a physiotherapist by background, in clinic I work as an ESP and review patients in the MND clinic, then the rest of my time I am supporting patients 

The professions I liaise with are 

GP: medication issues, hypersalivation, spasms, cramps

PT, mobility management, postural management, 

SLT: voice banking and swallowing / gastrostomy 

Dietetics: regarding weight loss / gastronomy

OT: Adaptation / equipment 

Local MND coordinators 

The effectiveness depends on the borough I am liaising some have specialist neuro team but some don't. Those with the neuro teams it is easier to liaise with as the MND patients tend not to be discharged and I can send a email after clinic with any issues, changes in function. Otherwise it is a referral to a team. Most GP practices you can email now which has made a great difference, the reality is they get so many clinic letter it is not always feasible they will act on the recommendations made. I email them and most times things are actioned quiet quickly.  OT is always an interesting one, you never know - nor do the patients if is was a NHS or SS OT they have seen. It can take me a little longer to track down the right person to liaise with. Sometimes people are under the care of both.  MND coordinators locally can be helpful, like all things it depends on the person I work with one person who is excellent and it feels like we are team working and the other leave passive aggressive messages and doesn't provide any useful information! 

Having online MDM has been a really big help as it is an opportunity for me to understand more about the teams I am liaising with.
