1) ensure GP's feel involved and supported
2) try to ensure we have the right people at the right time to support.
3) ensure the team communicates effectively and has a shared plan in which the person and others whom they wish to be are fully invovled. no decision about me without me.

5) ensure clients have a written escalation plan (beyond a respect form) which has contact details of whom to contact in what circumstances.

I was really struck with how involved Mark (in the video clip) felt in decisions about his care. If I could ensure that all the people i work with, felt like this, i'd be very pleased. in reality I think I am someway short of this. Not in all cases but certainly there is room for improvement.
Simon has provided a succinct overview of the key points that will improve patient care. Admittedly we may be aware of how we can improve services but this can be challenging during budget cut or when services are under time pressures.
