Instructions

Understanding how to support your team and receive support from them

Purpose

This activity will help you reflect on the information sharing and communication needed for effective MDT working, and consider how professionals can support one another to deliver coordinated, person centred care.

Task

Return to the MDT address book you created in week 2 and think about a person living with MND you have supported in practice. You may also wish to reflect on one of the patient experiences explored during week 3.

Consider the following questions:

  • What are the most useful pieces of information you would need from different MDT members to carry out your role effectively?
  • What information would you want other professionals to understand from your own professional perspective?

In the discussion board below:

  • state your professional role
  • choose three or four professions from the MDT or wider network
  • outline the information you would need from those professionals to support your practice effectively
  • reflect on what information colleagues may need from you in return.

As you read other learners’ posts, look for your own profession and consider whether the information colleagues value from your role matches your expectations.

Please contribute to the discussion below and engage with the reflections shared by other learners.
