When applying for my role as MND Specialist Nurse I was keen to understand how while working for the acute trust I would be able to link with the community trust/colleagues. This was well established prior to me starting my post.
After working with the MDT in acute stroke care I am familiar with working alongside other professionals and having conversations to ensure clear objectives are being set and communicated to the patient. It is more challenging with a variety of community colleagues however we do meet every 2 months for a MDM where we have a representative of each profession suggested within the NICE guidelines.
I ensure my patients are referred to the appropriate teams as per the NICE Guidelines; organisation of care.
I certainly feel reassured with multidisciplinary working, knowing that there is a professional supporting our MND caseload from all aspects of care.
I am keen to continue networking with other professionals, in other trusts to share ways of working.
