As my role as a assistant practitioner in the community, I have the privilege of visiting people in their own home. recognise the importance of good communication between the hospice team I work with and MND specialist team. GP'S, therapists and other professionals involved in a person's care. Sharing information and reporting any changes or concerns promptly helps ensure patients receive the right support at the right time. I feel it is important to look beyond the diagnosis and understand what matters most to each individual . Supporting someone at home allows me to see how their condition affects their daily life, independence and family.
This has reinforced my understanding that good organisation of care is not just about professionals working together, but about ensuring patients and families feel listened to. involved in decisions and supported throughout.
Hi Melanie, as a community-based OT I enjoyed reading your reflection as I also see such significant value in supporting people in their home environment, and advocating for people to receive additional supports so they can remain in their home for as long as safely possible if that is their goal.
