Examining your team

Purpose

This activity will help you explore communication within your MDT and consider how current approaches support effective care.

Task

Start by reading this document, which outlines one way of describing the key elements involved in providing effective care:

Structure, process and outcome

After reading, reflect on the following questions:

  • What communication channels are available to your team?
  • How does your team use these channels to support effective communication?
  • How well does this work in practice?
  • What changes could improve communication and help achieve better outcomes?

You may find it helpful to record your thoughts using the Word template or PDF template. You can save the completed Word template to your computer or print the PDF.

Finally, choose one aspect of communication and write a discussion post of approximately 300 to 400 words. Share the methods you currently use and suggest any possible improvements.

Once you have posted your response, read other learners’ contributions and reply to one or two posts.
